How to Sign Up for Duly MyChart

Duly MyChart gives you direct online access to information from your electronic medical record (EMR). It enables you to securely use the Internet to help manage and receive information about your health. You can sign up for Duly MyChart to:

  • Request and schedule physician office medical appointments.
  • Receive physician office appointment reminders.
  • View your health summary from the Duly MyChart electronic health record.
  • View test results.
  • Request prescription renewals.
  • Access trusted health information resources.
  • Communicate electronically and securely with your physician office medical care team.
  • While you cannot request hospital appointments through MyChart, you can view appointment information and patient instructions.

How to Sign Up for Duly MyChart

Registering for a new MyChart account can be completed in a few easy steps.

Step 1: Request an Activation Code

To receive an activation code for your own health record information, you may request an activation code at your doctor’s office or request an activation code online at mychart.duly.com/MyChart/Authentication/Login/signup.

To receive an activation code to view a family member’s health information, complete the Proxy Form. You may also mail or fax the Sign-Up and Proxy Forms to our Health Information Management (HIM) Department using the contact information that can be found on the forms.

Your activation code will be emailed to you within 72 hours of the completion of your form, coming from donotreply@mychartadmin.com.

NOTE: Activation codes can only be requested online for patients 18 or older. For minors or to receive access to another adult’s MyChart account, please complete the Proxy Access forms here.

Step 2: Complete Registration

Once you’ve received your authorization code, complete your Duly MyChart two-fac­tor Authentication setup here or at the link provided in your authorization code email.

Step 3: Log In!

You now have access to your online medical records as well as the ability to schedule appointments, message your providers and complete video visits even with the Duly MyChart mobile App
